Scope
Denied life-insurance claims, including disputes over alleged misrepresentation, concealment, suicide and contestability clauses, and missing beneficiary documents (under the Insurance Code, RA 10607)

Before you engage
A claim usually rises or falls on two things: what the policy actually says, and whether the proof of loss was complete and on time. Keep the full policy, the denial or settlement letter, your claim forms, the receipts and medical or repair records, and every message with the insurer or HMO. Do not sign a quitclaim or accept a final payment before counsel reviews it. The periods to file with the Insurance Commission or in court can be short, so do not let a denial sit. No honest lawyer can promise that a denied claim will be paid, and this firm will not. What it can do is test the denial against the policy and the Insurance Code, make the formal demand, and take the claim to the Insurance Commission or to court where that is the better path.

Not necessarily, but the answer depends on the policy and the reason given. A denial can rest on a real exclusion, on a contestable misrepresentation, or on a proof-of-loss gap that may still be curable. Bring the policy and the denial letter, and the firm will read the stated reason against what the policy and the Insurance Code (RA 10607) actually require, then tell you honestly whether there is a claim worth pressing rather than sell you a result.
A formal demand often moves a stalled file, and where it does not, the claim can go to the Insurance Commission for mediation or to a civil court for the proceeds. The law may also allow interest and damages where a valid claim is unreasonably refused or delayed, though that is decided on the facts and is never guaranteed. Keep the policy, the offer, and every message, and bring them in before you sign anything final.
Yes. HMOs are regulated by the Insurance Commission, so a refused pre-authorization, a "pre-existing condition" denial, or an unpaid bill can be raised by demand and, if needed, before the Commission. Bring your HMO contract or member guide, the denial or the hospital statement, and your medical records, and the firm will assess where the dispute stands.
